SMS Messaging Terms & Conditions
Effective Date: June 28,2024
The following terms and conditions apply to your use of the applicable text messaging services offered by Georgia Mobile Healthcare Serviced, LLC. On behalf of itself and its affiliates (“GMHS”). GMHS may update these terms at any time, without prior notice to you. Any updates to these terms will be effective as of the date listed on this page.
CONSENT FOR TEXT MESSAGING REMINDERS
I give permission consent to receive informational, conversational and promotional text messages from GMHS or others acting on the GMHS’s behalf. As part of this consent, You represent and warrant the following:
(1) GMHS or others acting on their behalf may send text messages in various formats and with various contents, including but not limited to, text messages about appointment reminders.
(2) You are the owner or authorized user of the mobile phone number identified below. You will notify us immediately if you are no longer the owner or authorized user of the mobile phone number identified below.
(3) You are solely responsible for any message and data charges associated with such text messages.
If You do not wish to receive text messages from GMHS or others acting on their behalf,
do not accept or sign this form.
GMHS Mobile Texts
The following terms and conditions (“GMHS Mobile Terms”) apply to your use of the Georgia Mobile Healthcare Service mobile text messaging services (“GMHS Mobile Texts”).
By Opting-In (defined below), you agree to be bound by these Terms.
GMHS Mobile Texts allow patients to receive informational text messages to their mobile phone from GMHS relating to appointments and other healthcare services. Message frequency varies. Message and data rates may be charged by mobile service providers
· How to Opt-In: You may opt-in to GMHS Mobile Texts by signing up on your patient registration consent form (“Opting-In”) or by texting “start or subscribe” to (770) 804-5250. By Opting-In, you consent to receive mobile text alerts using an automatic telephone dialing system. Consent to receive GMHS Mobile Texts is not required as a condition of purchasing any goods or receiving any services. By Opting-In, you are confirming that you are over the age of eighteen (18) and that you are providing us with a valid mobile number.
· Help Information: For additional information or general assistance, please contact us at (770)804-5250. For additional information or assistance relating to a specific GMHS Mobile Texts program, please text “HELP” to the number for the applicable GMHS Mobile Texts program(s) listed below:

· Stop Information: To stop receiving GMHS Mobile Texts from the program(s) listed below, please text “STOP” to the number for the applicable GMHS Mobile Texts program listed below:

You will receive a confirmation text. If you do not receive the confirmation text, you may still receive text messages from the applicable GMHS Mobile Texts program. Please text “STOP” to the applicable number above until you receive a confirmation text that you have opted out.
Privacy
At GMHS, we take your privacy seriously. We do not share or sell SMS opt-in data or consent, or phone numbers for the purpose of SMS. Any information we collect through GMHS Mobile Text (such as phone number) will be handled in accordance with our Privacy Policy.
